COVID-19 VACCINE CLINIC AT PHS - May 14 IS NOW FULL
THIS CLINIC IS NOW FULL - we are working to schedule another date with St. Luke's.
Parkland will be partnering with St. Luke’s University Health Network to provide a COMMUNITY COVID-19 VACCINE CLINIC next Friday, May 14, 2021, from 1:30 – 5:30 PM. The Pfizer vaccine will be offered to anyone 16 and older and is free of charge. The CDC meets on Wednesday, May 12, and may approve the vaccine for 12+. If they approve it by Thursday, May 13, at 12 noon, the 12-15 year-olds will be able to keep their requested appointments and be vaccinated at PHS on Friday.
There will be an early dismissal at Parkland High School at 1 PM (which was previously scheduled and included on the calendar). The clinic will begin at 1:30 PM and last until 5:30 PM. Anyone can attend and receive a free vaccine.
A Minor Acknowledgement Consent Form must be filled out for every child under the age of 18. Students who are 16 and 17 can receive the vaccine without an adult present as long as they bring the signed form (must be printed out and signed – no electronic submissions). Anyone <16 will need the printed form signed by the parent/guardian and must be accompanied by an adult. Anyone <18 year old should bring a school or photograph ID.
The Pfizer vaccine is a two-dose vaccine. The 2nd inoculation will occur at Parkland High School on June 4 from 3-7 PM.
NOTE: If a parent is registering a child, the parent should enter the child’s name and demographics, not the parent’s name and demographics.
NOTE: If you've already had the first dose somewhere else, you can't use this clinic and you need to return to your original site for your second dose.
